Coming October 2023: Aerostar – Exclusive with NIHCA

  • Must be an Aerostar employee
  • Eligible adult must be covered by the Aerostar health insurance
  • 8 visits = up to $30 reimbursement

Avera Health – Exclusive with NIHCA

  • Eligible adult must be enrolled on the Avera Health Employee Health Plan
  • Must be an Avera employee and/or spouse only
  • 8 visits = up to $20 reimbursement
  • One (1) card per Member

Blue Cross Blue Shield of North Dakota – Exclusive with NIHCA

  • Up to 2 people per household
  • Subscriber and subscriber’s spouse
  • 12 visits = redeemable for points
  • 9-Month program ~ Jan-Sept
  • One (1) card per Household

Fargo Public Schools – Exclusive with NIHCA

  • Up to 2 people per household
  • Subscriber and subscriber’s spouse; both must be covered under the District’s health insurance program
  • 12 visits = up to $20 reimbursement
  • Employee’ spouse adds an “S” at the end of the badge #

HealthPartners

  • These are HealthPartners general Frequent Fitness program requirements. Program requirements may vary by employer.
  • Up to 2 people per household; Must be 18 years or older
  • 12 visits = up to $20 reimbursement
  • One (1) card per Member

Medica

– Discontinued end of 2023

  • Each member must work out 8 or 12 visits/month, depending on their insurance policy; Members must be 18 years or older.
  • Medica provides up to a $20 credit with a maximum of 2 credits per month per family towards health club membership monthly dues.
  • One (1) card per Member

Midco – Exclusive with NIHCA

  • Employee + 2nd participating adult
  • 2 people per household; 18 yrs of age or older
  • 8 visits = up to $20 reimbursement
  • Dep ID # will be “C” for adult child and “S” for spouse

Northern Plains Insurance Pool – Exclusive with NIHCA

  • Employee Only
  • 8 visits = up to $20 reimbursement
  • One (1) card per Employee

PreferredOne

– Discontinued end of 2023

  • These are PreferredOne general Fitness Advantage program requirements. Program requirements may vary by employer.
  • Up to 2 people per household; Must be 18 years or older
  • 12 visits = up to $20 reimbursement
  • Some programs may vary, ask your employer for details
  • One (1) card per Member

Prime Health – Exclusive with NIHCA

  • Eligible Employee + 2nd Participating Adult
  • Twelve (12) visits = up to $20 reimbursement
  • One (1) card per household

PrimeWest – Exclusive with NIHCA

  • Eligible programs: PrimeWest Senior Health Complete (MSHO) and Prime Health Complete
  • as of 1/1/2024: Eligible programs will also include Minnesota Senior Care Plus (MSC+) and Special Needs BasicCare (SNBC)
  • Zero (0) visits = up to $20 reimbursement
  • 1 card per member

Sanford Health Plan – Exclusive with NIHCA

  • Up to 2 people per household
  • Subscriber and subscriber’s spouse
  • 12 visits = up to $20 reimbursement
  • One (1) card per Member

Sioux Falls School District – Exclusive with NIHCA

  • Up to 2 people per household; 1 card per Employee
  • Subscriber and subscriber’s spouse
  • Both the subscriber and spouse must be covered under the District’s health insurance program
  • 8 visits = up to $20 reimbursement

South Country Health Alliance – Exclusive with NIHCA

  • Senior Care Complete: 

    • No minimum (0) visit required to receive an up to $40 reimbursement p/mo
    • Must have a paid membership at your gym

     

  • AbilityCare:
    • No minimum (0) visit required to receive an up to $65 reimbursement p/mo
    • Must have a paid membership at your gym
  • MSC+, SharedCare & SingleCare*:

    • 4 (four) visits required to receive an up to $20 reimbursement p/mo

    *There is no limit per household, as long as each participant is enrolled during the respective month and is eligible to receive the discount.

UCare

  • Medicare Members:

    • No Minimum visits = up to $30 reimbursement
    • Must have a paid membership

    Individual Family Plan (IFP), UCare MinnesotaCare & UCare MA:

    • 12 visits = up to $20 reimbursement
